Make an impact! Interfaith Outreach is looking for candidates for a Case Manager position within our Permanent Supporting Housing program. This role is full-time (40 hrs/wk, benefits eligible).

What you will do: The Case Manager position will have two areas of focus: (1) supporting the housing stability of a small group of families who have experienced long-term homelessness and have complex barriers to maintain housing over time and achieve individual goals, and (2) supporting clients who are experiencing homelessness or housing instability to access resources and develop housing search plans.

Main responsibilities:

Permanent Supportive Housing (50%)

 

  • Recruitment and screening of potential residents
  • Welcoming families, setting up households and addressing basic needs
  • Problem solving and crisis management
  • Assessment, goal planning, connecting to resources and work over time to maintain housing stability

 

Housing Coaching (50%)

 

  • Maintain current information on housing and homelessness systems and resources
  • Establish relationships with local landlords to stay updated on openings in the area
  • Consult with team members on homelessness and housing issues
  • Consult with clients on accessing shelter
  • Support clients to create and implement housing search plans

Schedule requirements: The Case Manager will work primarily on-site in two housing communities located in Plymouth and Medina. A portion of the work can be done remotely. Must be willing and able to perform essential job functions through in-person, face to face contact as required, in accordance with health and safety guidelines.

Hours will generally take place Monday-Friday between 9 a.m. – 5:30 p.m. with the potential for some early evening hours.

Qualifications

Required Qualifications:

 

  • BSW or BA/BS in related field
  • Three years of experience working in a social services setting
  • Thorough understanding of the case management process
  • High level of computer proficiency
  • Commitment to equity and inclusion and ability to excel in a diverse environment
  • Capable of relating to and advocating for diverse client needs and interests
  • Ability to creatively problem solve and complete complex tasks
  • Excellent organization and time management skills
  • Ability to maintain professional boundaries
  • Strong interpersonal, verbal and written communication skills
  • Demonstrated ability to excel in a team environment

 

Preferred Qualifications:

 

  • MSW and five years of experience providing case management services to individuals and families
  • Social Work license
  • Experience working with Permanent Supportive Housing programs
  • Bilingual in English and Spanish, Somali or Russian
  • Knowledge of the social service network in west suburban communities
